Tele-Counseling Appointment

Complete the steps below to get started.

We offer tele-counseling appointments to offer the support you need in a format that is most convenient to you. If your clinician offers tele-counseling as a means of attending your appointment, you may do so through the platform, Microsoft Teams. Once your appointment is confirmed, you will receive an email from your clinician providing you with the appointment link.

Download the app now and be ready when your first appointment starts. You may download and join from your computer, tablet, or smartphone.

When completing the registration form, you will be asked to upload scanned copies of various documents and files. We recommend downloading the Adobe Scan app for your iPhone or Google/Android device to take and upload photos instantly from your phone as PDFs.

For current in-person clients who are transitioning to tele-counseling appointments, only step 2 needs to be completed.

Step 1

Complete the online registration form to start the process for booking your appointment.


After completing the registration form, download the Welcome Letter for introductory instruction.


Step 2

Fill out and submit the following online forms prior to your first appointment. 

Step 3

Fill out and submit the appropriate online Health History Form prior to your first appointment, as well as the HIPAA Acknowledgement & Authorization form.

Step 4

Optional: Complete the Release of Information Authorization form if you are needing your information sent to a third-party.
